INSURANCE POLICIES

Our clinic participates with Delta Dental, Medicare, most of the Blue Cross plans, as well as many of the other insurance plans. If you have questions regarding your coverage, please contact your insurance company directly.

We do ask that you provide both medical and dental insurance information at the time of your visit. We will submit either your dental or medical claim for your services, depending upon the classification of your services.

Please note that, although we may submit the insurance claim(s) for you, you are ultimately responsible for payment on your account. The clinic does not accept responsibility for the negotiation of insurance claims.

CREDIT POLICIES

After your initial consultation with the physician and prior to your surgery, you will be scheduled for a brief meeting with a patient care specialist to discuss the charges, your insurance benefits, payment arrangements and any other questions you might have regarding billing.

Initial consultation and related diagnostic procedure charges (i.e. x-rays, lab work, etc.) are due in full at the time of the consultation, unless prior arrangements have been made with our office.

If you have private insurance, a down payment of 20% of your charges is required on the day of surgery. You will be billed for any additional amount after your insurance has paid.

If you do not have insurance, payment in full is required on the day of your appointment. For your convenience, we accept Visa, MasterCard, Discover and American Express.

If you have had a consultation regarding dental implants and would like to schedule surgery, our treatment coordinator will discuss the payment policy with you. The full cost of the consultation and any x-rays will be due at the time of your initial visit. Elective surgeries, such as cosmetic facial procedures are rarely covered by insurance plans.

Account balances over 60 days old will be assessed a service charge regardless of insurance coverage. You are ultimately responsible for all account charges.
